2. Fill out completely. Ask your doctor for help.

3. Insert in plastic holder.

3.   4.  If you have completed a DNR, Health Care Proxy or have other important health care papers, insert copies in holder. 

5. Place in upper right corner of refrigerator.

6. Place VIAL OF LIFE magnet on the upper left hand comer on the outside of your refrigerator. Police, fire officers and ambulance attendants have been informed of this program. They will look inside the refrigerator for the VIAL containing your personal medical information.
